Google Forms (Advanced)
Once you are comfortable with using Google Forms to make simple surveys and quick quizzes, try your hand at using a feature of Google Forms to direct those answers your questions to different questions based on their answers. This is accomplished by adding Sections to your form and check marking "Go to section based on answer." This type of form requires a little more planning. It may be helpful to create a flow chart to help plan the path through the form. See sample below.
This starts with a question that leads to 2 choices. One choice gives a result. The other leads to another question. That question leads to 2 different results.
For this form, I will need to make 5 sections.
Section 1 - The title of this form is "Lamp Doesn't Work". The 1st question, "Lamp plugged in?" will be in Section 1. It's answer choices are "Yes" or "No".
Section 2 - "No" leads to Section 2 where the result "Plug in lamp." will be displayed. This section will need to go to the "Submit Form" section.
Section 3 - "Yes" will lead to Section 3 where the new question, "Bulb burned out?" will be displayed. It also has a "Yes" or "No" set of answers.
Section 4 - "Yes" for question 2 leads to Section 4 where "Replace bulb." is displayed. This section will need to go to the "Submit Form" section.
Section 5 - "No" for question 2 leads to Section 5 where "Repair lamp." is displayed. This section will need to go to the "Submit Form" section.
Submit section - All of my "Result" sections will need point to a hidden, but necessary "Submit" section. This is where the person filling out the form will give me their final answers.
